The person in this role is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs.* Location: Onsite - Milwaukee Hospital* PRN: Fill in for the needs of the team - could be up to 2-3 days per week.* Hours: 8:00 am - 4:30 pm* No evenings* No weekends**Experience:*** 1+ years of previous social work or related field experience required* Hospital experience preferred**Education:*** Master's Degree MSW required**Licenses and Certifications:*** Wisconsin Advanced Practice Social Work (APSW) certification* AND* **One of the following**:* License Social Worker Advance Practice (121) - State of Wisconsin* License Social Worker Clinical (123) - State of Wisconsin* License Social Worker Independent (122) - State of Wisconsin**Essential Functions:*** Understands and satisfies the needs of the neonatal, infant, toddler, pre-school, school-age, adolescent and/or adult patient in regards to their growth and development process.* Assists families in meeting their needs through: Short term, crisis oriented counseling; referrals to community agencies, programs and services; educating patients/families about illness, hospital procedures and community resources.* Helps plan for the future health care needs of the child whether or not admitted to the hospital.* Consults with other professional staff to assure that they understand the strengths and needs of patients and families.
