Qualifications

• This role requires a current RN license in Arkansas.
• Minimum one year in an acute care setting with geriatric-focused care.
• Ability to assess patient data, determine needs by age, and apply clinical judgment with digital health tools.

Responsibilities

• Provide hospital-level nursing care in the home with advanced assessments and interventions.
• Administer IV medications/infusions, complex wound care, and oxygen management as needed.
• Coordinate mobile diagnostics, equipment, and supportive services (PT, social work).
• Act as Transitional Care case manager for 30 days post-discharge to prevent readmissions.
• Collaborate with physicians/NPs and the interprofessional team via telemedicine and other technology.
