Job summary

Beth Israel Lahey Health, Inc. is seeking a Field Nurse to support the Hospital at Home program, delivering inpatient-level care to patients in their homes within a 25-mile radius of a BILH hospital.

This is a full-time, on-site position with some centralized virtual nursing from Charlestown. Responsibilities include protocol-driven care, coordination, and visits using biometric data and collaboration with the care team to ensure safe, evidence-based practices.

Qualifications

• Nursing Diploma or ASN required; BSN preferred.
• Massachusetts RN license and BLS certification required.
• Flexible and able to work across shifts in a dynamic home-based setting.

Responsibilities

• Provide protocol-driven assessment, care coordination and interventions.
• Deliver biometric data monitoring and respond per Hospital at Home protocols.
• Conduct audio/visual patient visits as directed by the care team.
• Promote patient-centered care and safety within the home setting.
• Collaborate with multidisciplinary team and participate in quality improvement.
