Job description

Reporting directly to the Hospital at Home Director of Nursing, the Hospital at Home Staff RN is responsible for providing high‑quality patient care to various patient populations within the BILH Hospital at Home program. The program delivers inpatient‑level care for patients in the comfort of their homes. The Staff RN is a field‑based nurse with some virtual nursing support from a centralized command center located in Charlestown. This is a fully on‑site position, and field assignments can be within a 25‑mile radius of any BILH hospital.

Job Description
Essential Duties & Responsibilities
  • Utilize monitoring, assessment, planning, intervention and critical thinking skills to care for acute‑care patient populations in the home.
  • Navigate patients and families across the continuum of care.
  • Monitor and respond to biometric data per Hospital at Home Command Center protocols.
  • Conduct audio/visual patient care visits as directed by the care team and/or protocol.
  • Ensure all interventions in the patient’s care plan are implemented efficiently with a patient‑centered approach.
  • Leverage the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high‑quality, evidence‑based care with clinical protocols as the foundation.
  • Adhere to the clinical protocols and be willing to obtain the necessary training to provide care within the context of evidence‑based care.
  • Promote professional practice and a culture of safety; willingness to engage in process improvement efforts.
Clinical Roles And Responsibilities
  • Provide protocol‑driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post‑operative care.
  • Exhibit sound clinical decision making and critical thinking skills.
  • Maintain and practice in accordance with nursing guidelines per state and hospital regulations.
  • Facilitate admissions to the Hospital at Home utilizing Hospital at Home guidelines.
  • Serve as a role model for professional nursing practice.
  • Utilize nursing knowledge to identify, prevent and/or solve complex acute and/or recurring patient care problems.
  • Share expertise with the multidisciplinary team and serve as a resource to staff in areas of expertise.
  • Support research efforts to advance knowledge and promote evidence‑based practice.
  • Perform other duties and responsibilities as assigned by the Senior Director of Nursing.
Quality And Safety Responsibilities
  • Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents.
  • Participate in quality and safety investigations and root cause analyses.
  • Take appropriate action to ensure patient safety as a result of any investigation.
  • Serve as a resource in quality improvement processes and techniques.
Organizational Requirements
  • Maintain strict adherence to the Lahey Hospital and Medical Center Confidentiality Policy.
  • Incorporate Lahey Hospital and Medical Center Guiding Principles, Mission Statement and Goals into daily activities.

Education

Minimum Qualifications: Nursing Diploma or ASN; BSN preferred.

Licensure, Certification, Registration
  • A current Massachusetts RN license.
  • BLS certification.

Pay Range

$37.00 - $95.74
