Job summary

Beth Israel Lahey Health is seeking a Hospital at Home Staff RN to deliver inpatient-level care at patients\' homes. The role is field-based with some centralized nursing from Charlestown, and is fully on-site with field assignments within a 25-mile radius.

The RN will perform protocol-driven assessments, coordinate care, monitor biometrics, and support a multidisciplinary team in a safe, patient-centered environment. Eligibility includes MA RN license and BLS; 3 years RN experience preferred.

Qualifications

• Current Massachusetts RN license.
• BLS certification.
• Three years RN experience.

Responsibilities

• Provide protocol driven assessment, care coordination and interventions within Hospital at Home for diverse patient populations.
• Monitor biometric data and respond to protocol guidance from the Hospital at Home Command Center.
• Coordinate admissions and discharges with the care team and support services.
• Promote patient safety and professional nursing practice; participate in quality initiatives.
• Collaborate with multidisciplinary teams and mentor colleagues as needed.
