Job summary

Advocate Health, expanding the Hospital at Home program at Navicent Medical Center, seeks an Acute Care Nurse Practitioner or Physician Assistant for a full-time hybrid role delivering inpatient-level care in patients’ homes and virtual settings.

The APP works with physicians and a multidisciplinary team to improve quality, access, and outcomes, performing histories, exams, orders, and care plans in a hybrid environment.

Qualifications

• Master’s Degree or DNP from an accredited NP program.
• Current RN and APN license in the applicable state.
• National board certification appropriate to specialty (ACNP/AGACNP/AGNP/PNP/PNP-AC).
• DEA license or eligibility for prescriptive authority.
• BLS certification with >90 days validity.

Responsibilities

• Performs initial and periodic histories, exams, and daily assessments.
• Records findings, plans of care, and interventions.
• Performs minor procedures within scope of practice.
• Writes prescriptions and administers medications.
• Orders and interprets labs and radiology tests.
• Determines and implements treatment plans.
• Documents encounters in the EMR and acts as patient advocate.
• Participates in interdisciplinary meetings and quality improvement.

Education

Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program

Current RN and Advanced Practice RN license in the applicable state

National board certification appropriate to specialty

DEA license eligible

Prescriptive authority eligibility

BLS certification (valid > 90 days)

Advocate Health is seeking an Acute Care Nurse Practitioner or Physician Assistant to join the Hospital at Home team at Navicent Medical Center. This full-time hybrid opportunity supports the expansion of the Hospital at Home program to Navicent Medical Center. The Hospital at Home APP provides specialized, patient-centered care to acute and chronically ill adult patients through a combination of virtual and in-home services. In collaboration with physicians and a multidisciplinary healthcare team, the APP delivers comprehensive inpatient-level care while helping expand an innovative care model designed to improve quality, access, and patient outcomes.

Essential Functions

• Performs initial and periodic medical histories, physical examinations, and daily patient assessments.
• Records findings of examinations, assessments, and plan of care.
• Performs minor medical and/or surgical procedures within scope of practice.
• Writes appropriate prescriptions and administers medications.
• Orders and interprets laboratory and radiology examinations.
• Determines and implements appropriate treatment plans.
• Documents patient encounters, interventions, and observations accurately in the electronic medical record.
• Acts as a patient advocate for the population served.
• Participates in interdisciplinary meetings, quality improvement initiatives, and development of policies and procedures as requested.

Physical Requirements

Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support up to 35 pounds while handling patients, medical equipment, and supplies. Must speak English fluently and possess the ability to react appropriately in stressful situations. Requires the ability to use sound judgment, communicate effectively, and work independently in patients’ homes and virtual care environments.

Education, Experience & Certifications

Nurse Practitioner

• Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program required.
• Current license to practice as a Registered Nurse and Advanced Practice Registered Nurse in the applicable state required.
• National board certification appropriate to specialty required (ACNP, AGACNP, AGNP, PNP, or PNP‑AC based on population served).
• Must have or be eligible for a DEA license.
• Eligible for prescriptive authority application through the applicable Board of Nursing or Medical Board as required.
• Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.

Physician Assistant

• Graduate of an accredited Physician Assistant program required.
• Current Physician Assistant license in the applicable state required.
• NCCPA certification required.
• Must have or be eligible for a DEA license.
• Eligible for prescriptive authority application through the applicable Medical Board as required.
• Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
