Qualifications

Responsibilities

• Perform comprehensive admission history and physical examinations for adult inpatients.

• Perform virtual inpatient rounding and follow-up assessments.
• Document all patient encounters and care plans in the EMR.
• Coordinate patient care with physicians, bedside APPs, nursing, and other teams.

Position Details

Virtual Hospital Medicine APP position

Swing work schedule: 7 on / 7 off

Rotating weekends and holidays

Perform virtual admissions, inpatient rounding, and cross-coverage responsibilities

100% inpatient adult medicine

Utilizes Advocate Health's virtual care platform to provide remote clinical support across multiple Greater Charlotte Region hospitals

Collaborative practice with Hospital Medicine physicians, bedside APPs, nursing, and interdisciplinary care teams

Epic electronic medical record

Required to live in one of the following states: North Carolina, South Carolina, Georgia or Alabama

Experience Required

• Graduation from an accredited Nurse Practitioner or Physician Assistant program
• Nurse Practitioners must hold a Master's Degree or Doctorate in Nursing Practice with Acute Care Nurse Practitioner (ACNP or AGACNP) board certification
• Physician Assistants must hold certification from the National Commission on Certification of Physician Assistants (NCCPA)
• Current license to practice as a Nurse Practitioner or Physician Assistant in North Carolina
• Current prescriptive privileges and DEA license
• BLS for Healthcare Provider (AHA) certification must be valid at least 90 days beyond start date
• Hospital Medicine, Internal Medicine, Critical Care, Emergency Medicine, or other acute inpatient experience preferred
• Demonstrated ability to work in a fast-paced, high-acuity environment with strong critical thinking and interpersonal skills
• Ability to perform thorough history and physical assessments
• Ability to function effectively within a multidisciplinary team
• Strong verbal and written communication skills

Job Summary

Experience utilizing Epic electronic medical record preferred. Specialty Care APPs provide concentrated services on a single system, caring for both acute and chronically ill patients. This Virtual Hospital Medicine position provides comprehensive inpatient care through virtual admissions, inpatient rounding, cross-coverage, and ongoing clinical management utilizing Advocate Health's virtual care platform. The APP collaborates closely with Hospital Medicine physicians, bedside providers, nursing, and interdisciplinary teams to deliver safe, efficient, high-quality, patient-centered care across multiple Advocate Health hospitals.

Essential Functions

• Perform comprehensive admission history and physical examinations for adult hospitalized patients.
• Perform virtual admissions and formulate appropriate treatment plans.
• Conduct virtual inpatient rounding and follow-up assessments.
• Record findings of examinations, assessments, and plans of care.
• Develop, implement, and modify treatment plans in collaboration with supervising physicians.
• Order, interpret, and evaluate laboratory, radiology, and other diagnostic studies.
• Write prescriptions and administer medications within the scope of practice.
• Monitor patient progress and respond appropriately to changes in clinical condition.
• Coordinate patient care with physicians, bedside APPs, nursing staff, consultants, pharmacy, rehabilitation services, case management, and ancillary departments.
• Document all patient encounters, assessments, interventions, and clinical decisions accurately within the electronic medical record.
• Act as a patient advocate for the population served.
• Attend, participate in, and contribute to department/practice meetings, quality improvement initiatives, and policy development as requested.
• Maintain compliance with Advocate Health policies, clinical practice guidelines, and regulatory requirements.
• Perform other duties as assigned within the scope of licensure and credentialing.

Physical Requirements

• Frequent sitting, computer use, speaking, hearing, reaching, and repetitive hand movements while utilizing virtual care technology.
• Ability to work for extended periods using multiple computer monitors and virtual communication platforms.
• Must possess intact senses of sight, hearing, smell, touch, and finger dexterity.
• Must speak English fluently and in understandable terms.
• Must be able to react and perform effectively in stressful clinical situations.
• Schedule may include swing shifts, weekends, holidays, and additional coverage as operationally needed.
