Advocate Health is seeking an Acute Care Nurse Practitioner or Physician Assistant to join the Hospital at Home team at Navicent Medical Center. This full-time hybrid opportunity supports the expansion of the Hospital at Home program to Navicent Medical Center. The Hospital at Home APP provides specialized, patient-centered care to acute and chronically ill adult patients through a combination of virtual and in-home services. In collaboration with physicians and a multidisciplinary healthcare team, the APP delivers comprehensive inpatient-level care while helping expand an innovative care model designed to improve quality, access, and patient outcomes.
Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support up to 35 pounds while handling patients, medical equipment, and supplies. Must speak English fluently and possess the ability to react appropriately in stressful situations. Requires the ability to use sound judgment, communicate effectively, and work independently in patients’ homes and virtual care environments.
Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program required.
Current license to practice as a Registered Nurse and Advanced Practice Registered Nurse in the applicable state required.
National board certification appropriate to specialty required (ACNP, AGACNP, AGNP, PNP, or PNP-AC based on population served).
Must have or be eligible for a DEA license.
Eligible for prescriptive authority application through the applicable Board of Nursing or Medical Board as required.
Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
Graduate of an accredited Physician Assistant program required.
Current Physician Assistant license in the applicable state required.
NCCPA certification required.
Must have or be eligible for a DEA license.
Eligible for prescriptive authority application through the applicable Medical Board as required.
Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
