The APP delivers inpatient-level care in patients' homes, collaborating with physicians and a multidisciplinary team, and helps expand the Hospital at Home program at Navicent Medical Center.

Qualifications

  • Master’s or DNP for NP route; RN and APN licensure required in the state.
  • DEA license and prescriptive authority eligibility required.
  • BLS certification with >90 days validity required.
  • NCCPA certification for PA route; active licensure as applicable.

Responsibilities

  • Take medical histories, perform exams, and daily assessments.
  • Record examinations, assessments, and care plans accurately.
  • Perform procedures within scope; write prescriptions and manage meds.
  • Order/interpret labs and imaging; implement treatment plans.
  • Document encounters in the EMR and advocate for patients.

This is a full-time hybrid position supporting the Hospital at Home program at Navicent Medical Center. The schedule consists of a mixed rotation of day shifts, including weekends, swing shifts, holidays, and weekend coverage based on operational needs. The role combines virtual patient care with in-home visits throughout the service area.

Pay Range

$51.05 - $76.60

Advocate Health is seeking an Acute Care Nurse Practitioner or Physician Assistant to join the Hospital at Home team at Navicent Medical Center. This full-time hybrid opportunity supports the expansion of the Hospital at Home program to Navicent Medical Center. The Hospital at Home APP provides specialized, patient-centered care to acute and chronically ill adult patients through a combination of virtual and in-home services. In collaboration with physicians and a multidisciplinary healthcare team, the APP delivers comprehensive inpatient-level care while helping expand an innovative care model designed to improve quality, access, and patient outcomes.

Essential Functions
  • Performs initial and periodic medical histories, physical examinations, and daily patient assessments.
  • Records findings of examinations, assessments, and plan of care.
  • Performs minor medical and/or surgical procedures within scope of practice.
  • Writes appropriate prescriptions and administers medications.
  • Orders and interprets laboratory and radiology examinations.
  • Determines and implements appropriate treatment plans.
  • Documents patient encounters, interventions, and observations accurately in the electronic medical record.
  • Acts as a patient advocate for the population served.
  • Participates in interdisciplinary meetings, quality improvement initiatives, and development of policies and procedures as requested.
Physical Requirements

Work requires walking, standing, sitting, lifting, reaching, bending, stooping, pushing, and pulling. Must be able to lift and support up to 35 pounds while handling patients, medical equipment, and supplies. Must speak English fluently and possess the ability to react appropriately in stressful situations. Requires the ability to use sound judgment, communicate effectively, and work independently in patients’ homes and virtual care environments.

Education, Experience & Certifications
Nurse Practitioner

Master’s Degree or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program required.

Current license to practice as a Registered Nurse and Advanced Practice Registered Nurse in the applicable state required.

National board certification appropriate to specialty required (ACNP, AGACNP, AGNP, PNP, or PNP-AC based on population served).

Must have or be eligible for a DEA license.

Eligible for prescriptive authority application through the applicable Board of Nursing or Medical Board as required.

Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.

Physician Assistant

Graduate of an accredited Physician Assistant program required.

Current Physician Assistant license in the applicable state required.

NCCPA certification required.

Must have or be eligible for a DEA license.

Eligible for prescriptive authority application through the applicable Medical Board as required.

Current Basic Life Support (BLS) certification required per policy guidelines, with an expiration date greater than 90 days from the date of beginning practice.
