Job summary

Responsibilities

• Collaborate with interdisciplinary treatment team to implement the treatment plan.
• Assist with financial-resource referrals, transportation, hospice, swing bed, nursing home and rehab referrals.
• Perform social work assessments considering bio-psychosocial factors and safety concerns.
• Document interventions in a timely manner and maintain confidentiality.
• Establish and maintain professional relationships with patients, families, staff, and resources.
• Participate in on-call rotations and adapt to changing workloads.
