Job summary

Qualifications

Responsibilities

• Coordinates placement of patients to ensure release to the least restrictive environment while maintaining safety.
• Consults with treatment team, mental health liaisons, patient, and family to determine needs and priorities.
• Writes or dictates progress notes documenting placement progress.
• Provides input during treatment team meetings.
• Ensures needs and resources are identified and addressed in treatment and discharge planning.
