Job summary

Stormont Vail Health is seeking an Ambulatory Social Worker to provide social work and case management services for patients, families, and support systems in ambulatory and inpatient settings.

The role involves conducting comprehensive assessments, developing treatment plans, and coordinating services across the health system, while supporting discharge planning and patient education. A licensed MSW or BSW is required, with preferred clinical experience and strong advocacy skills.

Qualifications

• Experience in customer service is required.
• Preferred clinical and/or medical social work experience.
• Preferred experience in group work, case work, and community methodologies.

Responsibilities

• Participates in patient admission process as hospital representative and advocate.
• Coordinates and facilitates services for patients and families across the Stormont Vail Health system.
• Performs social work assessments considering bio-psycho-social, cultural, developmental and spiritual factors.
• Develops case management or treatment plans based on assessments and multidisciplinary input.
• Addresses patient/family understanding and adjustment to illness to maximize intervention benefits.
• Establishes therapeutic relationships with patients and families and communicates clearly about status and progress.
• Initiates discharge planning and coordinates with internal and external teams.
• Develops and facilitates groups for patients and significant others as needed.
• Maintains confidentiality and adheres to professional ethics and standards.
