Job summary

Suncoast Hospice of Hillsborough, a member of Empath Health, invites applications for a Hospice Team Leader / Senior Staff Nurse (RN). This leadership-focused role coordinates patient care, supports the team, and ensures delivery of high-quality hospice services in Hillsborough County.

Workdays are Monday through Friday, 8am to 5pm. The ideal candidate holds an associate degree and an active Florida RN license, CPR certification, and preferable hospice supervisory experience.

Qualifications

• Associate’s degree (A.A.) or equivalent from two-year college or technical school; or one to two years related experience and/or training.
• Valid Florida RN license.
• CPR certification from American Red Cross or American Heart Association.
• Strong preference for supervisory experience.
• Hospice experience preferred.

Responsibilities

• Coordinates the nursing component of care on the IDT and provides coverage to team members as needed.
• Assist Care Team Manager with coordination of the Interdisciplinary weekly IDT meeting.
• Collaborates with the Care Team Manager to plan, implement and evaluate team goals and staff support.
• Coordinates 24/7 patient care coverage, including back-up and on-call planning.
• Reviews staff time sheets and patient care documentation for compliance with standards and regulations.

is seeking a caring and motivated Hospice Team Leader / Senior Staff Nurse (RN) to work with us a in Hillsborough supporting our patient care team. In this leadership-focus role, you will help coordinate patient care, support team members and ensure the delivery of exceptional hospice services to patients and families. Monday - Friday 8am to 5pm.

What you’ll Do:

• Coordinates the nursing component of care on the IDT. Provides coverage/assistance to team members as needed and patient acuity dictates to ensure continuity of care for all patients on the team. Assures assignments and redistribution of nursing caseloads. Coordinates team staffing needs with the Care Team Manager, other teams, and additional staffing resources.
• Assist Care Team Manager with coordination of the Interdisciplinary weekly IDT meeting.
• Collaborates with the Care Team Manager to provide assessment, planning, implementation and evaluation of team goals and support of staff.
• Coordinate the Team responsibility of providing 24 hours a day, 7 days a week patient care. Involves planning nursing back-up and on-call persons when necessary to ensure quality and continuity of patient/family care.
• Reviews and approves staff time sheets and patient care documentation as assigned. Assures compliance with all organizational standards including productivity, timeliness, and adherence to documentation guidelines. Periodic chart audits are utilized to ensure compliance with all organizational standards as well as State and Federal regulations and accreditation requirements.

What You’ll Need:

• Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; or one to two years related experience and/or training; or equivalent combination of education and experience.
• Licensure:Valid Florida license as a Registered NurseValid Florida State Driver’s License
• Certifications: Valid CPR certification from the American Red Cross or American Heart Association.
• Strong preference for at least one (1) year supervisory experience.
• Strong preference for minimum of two (2) years Hospice experience.
