Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team serving patients in Colorado. You will provide skilled nursing care in patients’ homes, collaborating with an interdisciplinary team to coordinate patient-centered plans of care and support families in difficult times.

Ideal candidates hold a nursing degree and current RN license, with CPR certification and robust computer skills to document and communicate effectively.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and valid driver’s license.
• Two years’ RN experience in clinical care; hospice/home health preferred.
• Current CPR certification.
• Proficient in computer use; Microsoft Office experience preferred.
• Strong interpersonal skills; ability to support patients and families.
• Ability to cope with family emotional stress and adapt to various lifestyles.

Responsibilities

• Administer skilled nursing care in clients’ homes; coordinate care with the interdisciplinary team, patient/family and referring agency.
• Coordinate care and communicate care plans with the team; ensure continuity of care across shifts and settings.

We are looking for a passionate On-Call Registered Nurse RN to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.

POSITION SUMMARY:

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

QUALIFICATIONS:

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
