Job summary

JSSA is seeking a Weekend Visit Nurse to provide compassionate hospice care in patients’ homes in Montgomery County. The role supports dignity and comfort at end of life and respects cultural traditions.

The Weekend Visit Nurse will perform initial patient assessments and serve as a clinical resource for families, patients, and referral sources. Schedule is every other weekend, Saturdays and Sundays, 8:30am–5pm.

Qualifications

• Active Maryland RN license.
• CHPN certification preferred.
• Minimum 3 years acute care and/or hospice experience.
• Excellent communication and customer service skills.
• Familiarity with Netsmart My-Unity EMR is a plus.
• A valid driver’s license and reliable transportation to Montgomery County.

Responsibilities

• Deliver hospice and palliative care in patients’ homes.
• Perform initial assessment of patient needs and coordinate with family.
• Facilitate end-of-life conversations and decisions with family or caregiver.
• Represent the organization in communications with patients, families, and physicians.
• Work every other weekend, Saturdays and Sundays 8:30am–5pm.

Job description

Our Hospice team has immediate openings for part-time and PRN Weekend Visit Nurses. You will work for one of the top Hospice programs in Montgomery County providing patients and their families with exceptional end-of-life care that supports dignity and comfort for individuals, peace of mind to families, and honors cultural traditions and customs.

Functions:

The Weekend Visit Nurse will deliver quality hospice and palliative care services to patients in their place of residenceS/he will perform initial assessment of patient’s needs, serve as a clinical resource for referral sources and patients/family members, facilitate end of life conversations and decision-making with the family or caregiver. Additionally s/he will represent our organization when communicating with patients, families, and physicians.Schedule is every other weekend, Saturdays and Sundays from 8:30am-5pm.

Position Qualifications:

An active State of Maryland RN license, CHPN certification preferred, and a minimum of 3 years of acute care and/or hospice experience. Prior Hospice experience preferred. Demonstrated knowledge and skills necessary to provide meaningful care to communicate with the terminally ill. Excellent communication and customer service skills; attention to detail; strong organization skills, and proficient computer skills. Familiarity with Netsmart My-Unity electronic medical records a plus. A valid driver’s license and reliable, independent transportation to provide care to hospice patients in Montgomery County. Must be available to work the assigned weekend shift.
