Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking a Weekend Hospice Visit Nurse to provide expert clinical care during weekends. You’ll deliver compassionate, patient-centered care in patients’ homes and other approved settings, with autonomy and support from an interdisciplinary team.

Weekend flexibility, competitive hourly pay, and mileage reimbursement are offered, along with a path to grow within Minnesota’s expanding hospice organization.

Qualifications

• Active Minnesota RN license.
• Hospice experience preferred or experience in oncology, palliative care, ICU, home health, or end-of-life care.
• Strong assessment, communication, and critical thinking skills.
• Ability to work independently in a field-based role.
• Reliable transportation and a valid driver's license.

Responsibilities

• Complete routine and PRN hospice visits throughout your assigned territory.
• Assess patients, manage pain and symptoms, provide wound care, and respond to changes in condition.
• Educate and support patients, caregivers, and families with compassionate communication.
• Coordinate care with physicians, social workers, chaplains, hospice aides, and the interdisciplinary team.
• Complete accurate, timely EMR documentation.
