Job summary

Emmanuel Hospice is seeking a compassionate Part-Time Weekend RN to join our interdisciplinary hospice care team in Grand Rapids, MI. The role provides skilled nursing care, education, and support to patients and families across multiple care settings during weekend hours from 7am-7pm.

The position entails admissions assistance, assessments, treatment planning, and ensuring continuity of care within the service area. Weekend shifts are Sat/Sun or every other Sat/Sun (12hr).

Qualifications

• Valid Registered Nurse license in Michigan.
• Minimum 2 years of nursing experience; prefer hospice or acute care.
• Experience with Electronic Medical Records (EMR).

Responsibilities

• Conduct initial and ongoing assessments of patients’ physical, psychological, social, and spiritual needs.
• Develop, evaluate, and update individualized treatment plans with the interdisciplinary team.
• Provide skilled nursing care including wound care, IVs, blood draws, and catheter care.
• Educate patients and families on symptom management and hospice care principles.
• Coordinate with physicians and team members to deliver end-of-life care.
• Assist with bereavement support and provide emotional support to families.
• Travel within service area to patient homes and facilities.

About the Role:

Emmanuel Hospice is seeking a compassionate and dedicated Part-Time Weekend RN to join our interdisciplinary hospice care team. This role provides skilled nursing care, education, and support to patients and families across multiple care settings during the weekend hrs from 7am-7pm. As a part time nurse, you will assist the nursing team with admissions, assessments, treatment planning, and interventions, while ensuring continuity of care across our service area.

• Schedule: Sat/Sun, or Every other Sat/Sun (12hr) shifts 7am – 7:30pm

Key Responsibilities:

• Conduct initial and ongoing assessments of patients’ physical, psychological, social, and spiritual needs
• Develop, evaluate, and update individualized treatment plans in collaboration with the interdisciplinary team
• Provide skilled nursing care including wound dressings, IVs, blood draws, and catheter care
• Educate patients, families, and caregivers on symptom management, disease processes, and the hospice philosophy
• Coordinate with physicians, hospice providers, and interdisciplinary team members to deliver exemplary end-of-life care
• Assist with bereavement counseling and provide emotional support to families and caregiversPerform urgent need visits and provide post-mortem care, including pronouncement of death and family support
• Supervise and evaluate Hospice Aide services, including periodic on-site visits
• Ensure compliance with Emmanuel Hospice policies, procedures, and regulatory standards
• Travel within the service area to patient homes and healthcare facilities

Qualifications & Experience:

• Valid Registered Nurse license in the State of Michigan
• Minimum 2 years of nursing experience required; 1 year in acute care or hospice preferred
• Experience with Electronic Medical Records required
• Strong communication and collaboration skills

Physical Requirements:

• Ability to push, pull, move, and/or lift a minimum of 50 pounds
• Comfortable working in varied environments, including patient homes and healthcare facilities
• Frequent travel required within the Emmanuel Hospice service area
