Job summary

Emmanuel Hospice in Grand Rapids, MI is seeking compassionate per-diem Administrative Support professionals to join our care team and provide meaningful end-of-life support. You’ll provide care and support to patients and families on varied schedules, triage after-hours needs, and collaborate with an interdisciplinary team to develop and implement plans of care.

Expect 2 shifts a month, 2 weekend shifts per quarter, and 1 holiday per year, with flexible scheduling and a supportive, inclusive

Qualifications

• License/Certificated in the state of MI
• Home Health, Assisted living or Skilled nursing facility, Long-term care, Nursing Home or Rehab

Responsibilities

• Provide care and support to patients and families during a variety of different schedules
• Triage and respond to after-hours patient needs via phone and in-person visits
• Collaborate with an interdisciplinary team to develop and implement plan of care
• Deliver exceptional, holistic care that honors the end-of-life journey
• 2 shifts a month
• 2 weekend shifts per quarter (every 3 months)
• 1 holiday a year
