Job summary

Patriot Hospice is seeking a Registered Nurse Case Manager for weekend shifts to coordinate hospice care with patients and families across home settings. The role includes start-of-care evaluations, SIA visits, triage, and ongoing assessments to ensure quality patient outcomes.

You will document findings, participate in IDG meetings, and support on-call coverage as required. Candidates should be a graduate of an accredited Nursing program, hold an RN license in the state, and have hospice

Qualifications

• Graduate of an accredited School of Nursing.
• Licensed RN in the state.
• Hospice experience preferred (1+ year).

Responsibilities

• Coordinate hospice care with patients, families, and the care team.
• Perform start-of-care evaluations and SIA visits.
• Triage patient calls and complete home visits as needed.
• Assess patient needs and monitor service effectiveness.
• Document clinical notes accurately and timely.
• Participate in IDG and case conferences.
• Handle on-call duties including holidays per policy.

This position is worked Friday-Sunday 8:00am-8:00pm

Territory: We are looking to fill roles in Trumbull, Mahoning, and Columbiana Counties.

As a Weekend RN, you will:

• Coordinate hospice care by working with all team members as well as patients and family members.
• Do start of care evaluations and Service Intensity Add-On (SIA) visits.
• Triage patient calls and complete visits as needed.
• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice care along with informational visits as needed.
• Submit complete and accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Perform on-call duties (including holidays) per policy and agency need.
• All other duties as assigned.

Qualifications:

• Must be a graduate from an accredited School of Nursing
• Must be licensed in the State as a Registered Nurse
• One or more years of experience in hospice preferred
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a criminal background check
• Must have a current CPR certification

Job Conditions:

• Must have a driver's license and be willing and able to drive to patient's residences
• The ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care
• Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients
• *This position is paid at a competitive hourly rate with the opportunity to earn extra money by picking up visits at our Per Diem rate*
