Job summary

Patriot Hospice in Ohio is seeking a Registered Nurse Case Manager to coordinate hospice care with patients and families, conduct start of care evaluations and SIA visits, triage calls, and participate in IDG/case conferences. Weekend shifts and on-call duties are included.

The role requires an active Ohio RN license, at least one year of hospice experience, and a willingness to drive to patients' homes. Competitive hourly pay with per diem opportunities and benefits are offered.

Qualifications

• Graduate from an accredited School of Nursing.
• Must be licensed in the State as a Registered Nurse.
• One or more years of hospice experience preferred.
• Must have knowledge of Medicare and Medicaid guidelines.
• Must have a criminal background check.
• Must have a current CPR certification.

Responsibilities

• Coordinate hospice care by working with all team members, patients, and family members.
• Perform start of care evaluations and SIA visits.
• Triage patient calls and complete visits as needed.
• Evaluate the effectiveness of hospice services for patient and family on an ongoing basis.
• Submit complete and accurate clinical notes promptly.
• Participate in IDG and case conferences, and perform on-call duties as required.

This position is worked Friday-Sunday 8:00am-8:00pm

As a Weekend RN, you will:

• Coordinate hospice care by working with all team members as well as patients and family members.
• Do start of care evaluations and Service Intensity Add-On (SIA) visits.
• Triage patient calls and complete visits as needed.
• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice care along with informational visits as needed.
• Submit complete and accurate clinical notes in a timely manner.
• Participate in IDG and case conferences.
• Perform on-call duties (including holidays) per policy and agency need.
• All other duties as assigned.

Qualifications:

• Must be a graduate from an accredited School of Nursing
• Must be licensed in the State as a Registered Nurse
• One or more years of experience in hospice preferred
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a criminal background check
• Must have a current CPR certification

Job Conditions:

• Must have a driver's license and be willing and able to drive to patient's residences
• The ability to access patients' homes which may not be routinely wheelchair accessible is required. Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient care
• Physical activities will include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients
