Job summary

Redcrest Hospice, part of the Alternate Solutions Health Network family, seeks an experienced RN to provide end-of-life care to patients at Ball Memorial Hospital with opportunities to work in the field. Shifts are 3 x 12 hours (Sat, Sun, Mon, 8:00 AM–8:00 PM) with holiday and after-hours rotation.

You will lead one-on-one patient care, work at the top of your license, and collaborate with an interdisciplinary team to deliver compassionate, quality care to patients and their families.

Qualifications

• RN license in the state of employment.
• Minimum one-year RN experience in an acute care setting preferred.
• Hospice experience is desired.
• Valid driver’s license and auto insurance in your name.

Responsibilities

• Plan of Care: evaluate treatment and condition; tailor the plan to patient needs.
• Minimize Patient Risk: update doctor orders and administer medications as prescribed.
• Continuity of Care: coordinate multidisciplinary care and handle admissions/transfers/discharges.
• Family Educator/Advocate: counsel family on disease progression and include them in care planning.
• Policies: complete clinical documentation per agency protocol and Medicare guidelines.
• Operations: oversee care provided by nurses aides and LPNs; attend trainings.

Company: Redcrest Hospice

Schedule: 3 x 12-hour shifts weekly (Saturday, Sunday, and Monday, 8:00 AM to 8:00 PM). Holiday rotation and after-hours rotation as needed.

Location: Primarily supporting General Inpatient Services at Ball Memorial Hospital but will occasionally do field as well.

You will utilize your clinical skills in providing end of life care as directed by an attending physician. HOW YOU'LL WORK

You'll oversee the overall care of the patient from beginning to end of life. You will observe and monitor the patient to determine if the level of care being provided meets the patients’ needs. You will work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state.

MAJOR AREAS OF RESPONSIBILITY

• Plan of Care Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change.
• Minimize Patient Risk Update all doctor orders in patient chart and administer medication as prescribed by the physician.
• Continuity of Care Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Family Educator/Advocate Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
• Policies Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Operations Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.

HARD & SOFT SKILLS

Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions.

REQUIREMENTS

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
