Job summary

Alternate Solutions Health Network is seeking a Hospice Registered Nurse Case Manager (RNCM) in the Lafayette, IN area. You will work with an interdisciplinary team to deliver end-of-life care, manage symptoms, and support patients and families through a difficult time.

The role requires an active RN license, hospice experience, and strong communication. A valid driver’s license is required; clinical documentation and team coordination are essential parts of the job.

Qualifications

• Registered Nurse with current license in the state of employment.
• Minimum one-year experience as an RN in an acute care setting is preferred.
• Hospice experience is desired.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.

Responsibilities

• Symptom Management: work with the attending physician and Medical Director to manage patient symptoms.
• Plan of Care: evaluate and adjust the plan of care as needs change.
• Continuity of Care: coordinate admissions, transfers and discharges.
• Family Educator/Advocate: educate and involve family in care planning.
• Documentation: complete clinical documentation per agency and Medicare guidelines.
• Operations: supervise total care by nurse aides and LPNs; attend trainings and meetings.

Schedule: Monday-Friday | 8:00 AM-4:30 PM | On-call rotation (as back-up only)

As a Hospice Registered Nurse Case Manager (RNCM), you will be able to help patients and their families navigate the most intimate and difficult time of one’s life. Using hospice best-practice care delivery approaches, you will have all the tools and resources needed to ensure an excellent end-of-life experience for your patients. WHAT YOU WILL LOVE DOING

• Symptom Management: Through a thorough assessment, you will work closely with the patient’s attending physician and the hospice Medical Director to ensure every patient’s symptoms are well managed.
• Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and adjust as needs change.
• Continuity of Care: Manage interdisciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in development of plan of care. Listen to patient and family members.
• Documentation: Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Operations: Oversee and supervise total care of patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.

Compassionate communicator with a positive attitude. Patience is a virtue when working with patients, families, physicians, and coworkers. Attention to detail is critical, as is being observant and following directions.

REQUIREMENTS
