Job summary

JourneyCare Hospice is seeking a Weekend Admission RN to join our Illinois team. This role supports patient intake by performing pre-admission evaluations, educating families, obtaining informed consent, and initiating the admission process with compassionate, patient-centered care.

The ideal candidate is a graduate from an accredited RN program, licensed in Illinois, CPR certified, and with strong judgment and communication skills.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR Certification.
• 1 year of Hospice experience, preferred.
• Ability to make independent decisions when circumstances warrant.
• Ability to deal tactfully with patients, family members, visitors, agency personnel, and the general public.
• Knowledge of quality assessment and assurance procedures.

Responsibilities

• Perform pre-admission evaluations for the appropriateness of services.
• Initiates the admission process by educating and informing patients/families of its services.
• Obtains informed consent
• Provides nursing assessment, initial plan of care, and intervention(s) that optimize patient and family-directed comfort and quality of life.
