Job summary

Hartford HealthCare at Home, the leading homecare provider in Connecticut, seeks a dedicated RN to support patients and families in end-of-life care. You will perform comprehensive assessments and collaborate with an interdisciplinary team to deliver compassionate, person-centered care in the patient's home.

This role requires CT RN license, at least 1 year of RN experience (homecare/hospice preferred), valid driver's license, and computer proficiency.

Qualifications

• Licensed CT Registered Nurse with CT RN license.
• Minimum 1 year RN experience; homecare or hospice preferred.
• Valid driver's license and reliable transportation.
• Computer proficiency required.

Responsibilities

• Support our patients, families, and loved ones, as they go through their end of life journey
• Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies
• Be a member of an active interdisciplinary team that addresses patients' needs holistically
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
