Job summary

Hartford HealthCare at Home is seeking a Hospice Visiting Registered Nurse to support patients and families in the end-of-life journey. The RN will perform comprehensive assessments, establish symptom-management goals, and work within an interdisciplinary team to deliver holistic care in the patient’s home.

The role requires a CT RN license and at least one year of RN experience (homecare or hospice preferred), plus a valid driver’s license and solid computer skills for documentation and care

Qualifications

Active CT Registered Nurse license.
Minimum of 1 year RN experience; homecare or hospice experience preferred.
Valid driver’s license and reliable transportation required; computer proficiency required.

Responsibilities

Support patients and families through their end-of-life journey.
Perform comprehensive assessments to establish goals for symptom management and end-of-life care with effective case management strategies.
Be a member of an active interdisciplinary team that addresses patients’ needs holistically.
Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
Utilize state-of-the-art technology to document skilled services provided.
