Hartford HealthCare at Home is seeking a Hospice Visiting Nurse (RN) in Connecticut to support patients and families at end of life. You will perform comprehensive assessments, establish goals for symptom management, and participate in an interdisciplinary team to address patient needs holistically.

Vetted candidate will possess a CT RN license and at least one year of RN experience, preferably in homecare or hospice, along with a valid driver's license and strong computer skills.

Qualifikationen

• Registered Nurse with CT RN license
• One year of RN experience; Homecare or Hospice experience preferred
• Valid driver's license and reliable transportation required
• Computer proficiency required

• Perform a comprehensive assessment to establish goals for symptom management and end of life care with effective case management strategies
• Be a member of an active interdisciplinary team that addresses patients needs holistically
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families.
• Utilize state-of-the-art technology to document skilled services provided.
