Job summary

Hartford HealthCare at Home in Connecticut is seeking a Registered Nurse to support patients and families in end-of-life care within the home setting. You will perform comprehensive assessments to establish goals for symptom management and coordinate with an interdisciplinary team.

The role requires an active CT RN license, at least one year RN experience (homecare or hospice preferred), a valid driver's license, and computer proficiency. We offer a Nurse Residency program and strong benefits.

Qualifications

• Registered Nurse with valid CT RN license.
• One year of RN experience; Homecare or Hospice experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.

Responsibilities

• Support patients, families, and loved ones through the end-of-life journey.
• Perform a comprehensive assessment to establish goals for symptom management and end-of-life care with effective case management.
• Be a member of an active interdisciplinary team addressing patients’ needs holistically.
• Develop and maintain a therapeutic relationship through effective communication with patients and families.
• Utilize state-of-the-art technology to document skilled services provided.
