Qualifications

• Current RN license required.
• Hospice and/or Home Care experience preferred.
• New grads will not be considered.

Responsibilities

• Provide quality, safe hospice nursing in home or assisted living settings.
• Develop, implement and revise the plan of care with input from the care team.
• Coordinate care and communicate with other health professionals to address needs.
• Supervise and support LPNs and home health aides and conduct visits.
• Educate clients and families on home nursing procedures and care needs.
