Job summary

Butte Home Health & Hospice is seeking a compassionate Licensed Vocational Nurse (LVN) to join our Hospice team in a full-time, predominantly office-based role in Chico, CA. The LVN will provide telephone triage, assist with clinical documentation, and support the interdisciplinary team while maintaining strong patient and family communication.

This office-based position includes field training to perform hospice visits as needed, with a Saturday shift and weekday coverage.

Qualifications

• Current California LVN license.
• Strong communication and telephone skills.
• Excellent organization and follow-through.
• Comfortable communicating with patients and families during sensitive situations.
• Ability to recognize and appropriately escalate changes in patient condition.
• Strong teamwork and collaboration skills.
• Hospice, home health, skilled nursing, or other clinical experience preferred.
• Valid driver's license and ability to perform patient visits when needed.

Responsibilities

• Provide telephone triage for hospice patients and caregivers.
• Respond to patient and family calls and help identify clinical needs or changes in condition.
• Communicate concerns to the appropriate RN Case Manager or other clinical team member.
• Assist with coordinating medications, supplies, equipment, and other patient needs.
• Support physicians, pharmacies, facilities, and members of the interdisciplinary Hospice team.
• Assist with clinical documentation and other office-based nursing duties.
• Help coordinate changes in patient needs and ensure timely follow-up.
• Support the field team with day-to-day clinical and operational needs.
• Maintain familiarity with hospice policies, procedures, and patient care standards.

What You'll Do

• Provide telephone triage for hospice patients and caregivers
• Respond to patient and family calls and help identify clinical needs or changes in condition
• Communicate concerns to the appropriate RN Case Manager or other clinical team member
• Assist with coordinating medications, supplies, equipment, and other patient needs

Office-Based - With Field Training

This is a full-time office position, but the LVN will also be trained and maintain competency to perform hospice patient visits.

Although the primary responsibility is office-based, occasional field visits may be needed to support patient care, staffing needs, or changes in patient condition.

Schedule

• Full-time position

• Saturday shift is a requirement (other days may be negotiated)
• Schedule will include weekday office coverage in addition to Saturday
