Job summary

Sunbright Home Health & Hospice in Pleasanton, CA seeks an LVN After-Hours Triage Runner to provide compassionate, timely nursing care after hours, weekends, and holidays. Per Diem position supporting the clinical team with urgent visits and continuity of care.

The role requires CA LVN licensure, CPR, 1+ year nursing experience (home health/hospice preferred), and a willingness to travel within the service area. Per diem shifts include evenings and holidays.

Qualifications

• Graduate of an accredited LVN program.
• Current, active LVN license in California.
• Minimum one year of nursing experience; Home Health/Hospice preferred.
• Current CPR (BLS) certification.
• Valid California driver’s license and reliable transportation.

Responsibilities

• Respond to after-hours calls, triage patient needs, and determine level of intervention.
• Conduct home visits for symptom management, medication administration, wound care, or urgent needs.
• Communicate with the on-call RN Case Manager, Hospice Medical Director, or Administrator about patient conditions.
• Document all patient interactions and interventions in the EMR.
• Collaborate with interdisciplinary team to ensure continuity of care and handoffs.
• Educate patients and families on medications, symptom management, and emergency procedures.
• Maintain readiness to travel within the service area to address patient needs timely.
• Adhere to company policies and regulatory standards; participate in on-call rotations.
• Attend staff meetings as required.

Position Summary:

The LVN After-Hours Triage Runner provides compassionate, timely, and professional nursing care to home health and hospice patients during after-hours, weekends, and holidays. This position supports the clinical team by responding to patient and family needs, performing urgent visits, and ensuring continuity of care after regular business hours.

Essential Duties and Responsibilities:

Respond promptly to after-hours calls, triage patient needs, and determine the appropriate level of intervention.

Conduct home visits as needed for symptom management, medication administration, wound care, or other urgent clinical needs.

Communicate with the on-call RN Case Manager, Hospice Medical Director, or Administrator regarding patient conditions and changes.

Document all patient interactions, assessments, and interventions accurately and timely in the electronic medical record (EMR).

Collaborate with interdisciplinary team members to ensure continuity of care and smooth handoffs to the day team.

Educate patients and families regarding medication use, symptom management, and emergency procedures.

Maintain readiness to travel within the service area to address patient needs in a timely manner.

Adhere to all company policies, procedures, and regulatory standards, ensuring compliance with state and federal guidelines.

Participate in on-call rotation and staff meetings as required.

Qualifications:

Education: Graduate of an accredited Licensed Vocational Nurse (LVN) program.

Licensure: Current, active LVN license in the State of California.

Experience:

Minimum one (1) year of nursing experience preferred.

Home Health and Hospice experience strongly preferred.

Certifications:

Current CPR (BLS) certification.

Valid California driver’s license and reliable transportation with current auto insurance.
