Job summary

MEP Health is seeking an experienced Hospice RN On Call Triage/Float to provide after-hours clinical support for hospice patients and families. You will manage phone triage, assess symptoms, and coordinate urgent home visits, floating across multiple teams to ensure seamless patient care.

Collaborating with interdisciplinary team members, you will update care plans in real time based on patient needs, prioritizing comfort and safety in a homecare and hospice setting.
