Job summary

CARIN Alliance is seeking a Hospice RN on-call triage/float to provide after-hours triage for hospice patients in their homes and, when needed, perform field visits to assess patient conditions.

The role requires an RN license and strong triage skills, with a focus on compassionate, evidence-based care and coordination with the interdisciplinary team. Occasional in-person visits support patient care as needed.

Qualifications

• RN license required.
• Strong assessment and triage skills.
• Excellent communication and compassionate patient care.
• Ability to work independently and collaboratively.
• Comfort with documentation and electronic health records (EHR).

Responsibilities

• Provide after-hours phone triage for hospice patients in their homes.
• Conduct occasional field visits to assess patient conditions.
• Assess symptoms, prioritize care needs, and coordinate with the interdisciplinary team.
• Support families during critical moments with compassionate, evidence-based care.
• Ensure timely interventions and document care in the EHR.
• Float to support in-person visits as needed.
