Job summary

Jobtailor is seeking a Registered Nurse skilled in hospice care to perform comprehensive patient assessments, coordinate start-of-care visits, and educate patients and families about end-of-life care in Missouri. The role involves collaboration with hospital and facility teams to ensure safe discharges and appropriate levels of care.

Responsibilities include DME ordering, care coordination, and ongoing communication with clinical staff to minimize readmissions.

Qualifications

  • Associate degree from an accredited School of Nursing.
  • Minimum of two years of experience working as a Registered Nurse, including acute care or oncology experience.
  • Hospice experience preferred.
  • Experience with Microsoft Office Suite, EPIC, and HCHB preferred.
  • Able to transfer patients according to proper body mechanics to promote safety.
  • Ability to accept different lifestyles, cultures, beliefs, and values.
  • Proficiency with computers.
  • Successful completion of a drug screen prior to employment.
  • Legally authorized to work in the United States.

  • Determine hospice eligibility and appropriate level of care.
  • Communicate with the clinical team regarding complex, chronically ill patients to help prevent re-hospitalizations.
  • Partner with field staff and leaders to support patients.
