Job summary

Jobtailor is seeking a compassionate Registered Nurse to assess hospice patients and families, monitor conditions, and collaborate with a care team in home settings around Shawnee, Oklahoma. Qualifications

• RN license in good standing in Oklahoma.
• Graduate of an accredited nursing program.
• 1+ year clinical nursing experience.
• Home health experience preferred.
• Ability to lift up to 50 pounds and travel between patient homes.
• Valid driver's license or reliable transportation.
• Experience in hospice or home health environment preferred.

Responsibilities

• Assess hospice patients and families to identify needs.
• Monitor patient condition and communicate changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff on Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff.
• Provide compassionate support at the time of death and complete bereavement assessments.

Job description

Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs
Monitor patient condition and communicate significant changes, concerns, or safety issues to the physician and care team
Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care
Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings
Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers

Requirements

• Current State license as a Registered Nurse
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting
• Home Health experience is preferred, but not required
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations
• Experience in a hospice or home health environment is preferred
