Job summary

Enhabit Home Health & Hospice is seeking a dedicated professional to represent the branch in external transitional care activities and to promote our services. The role requires applying clinical guidelines to create patient-centered transition plans and acting as a public education ambassador for our care offerings.

The ideal candidate has at least one year in home care, proper state licensure, and comfort with mobile technology to support care coordination and compliance.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year of experience in home care.
• Must have demonstrated knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to promote Enhabit services.
• Integrate clinical guidelines and protocols to develop patient-centered transition plans.
• Serve as a public awareness representative and educate about provided services.

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient‑centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
