Job summary

Enhabit Home Health & Hospice is seeking a qualified healthcare professional to lead branch-level transitional care initiatives in the community. The role focuses on educating patients, families and providers about available home health services and coordinating transitions to ensure quality outcomes.

The position requires a state license, at least one year in home care, and familiarity with regulatory guidelines. Strong communication and technology skills are essential in a fast-paced omgeving.

Qualifications

• Must be a graduate of an approved school of clinical education.
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws and regulatory guidelines governing hospice operations.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible for education related to provided services.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
