Job summary

Enhabit Home Health & Hospice is looking for dedicated colleagues to join our team in Temple, Texas. We deliver compassionate, patient-centered home-based care and support clinicians, patients, and families across the community.

The successful candidate will be licensed in Texas, graduate of an approved clinical program, with at least one year in home care, knowledge of relevant laws, and strong technology skills to manage documentation and mobile devices.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they practice.
• At least one year of experience in home care.
• Knowledge of federal, state, and local laws governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to apprise them of Enhabit services.
• Develop patient-centered transition plans that promote quality and efficiency by integrating clinical guidelines and metrics.
• Serve as a public awareness representative and education resource for provided services.

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Qualifications

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
