Job summary

Enhabit Home Health & Hospice in the United States seeks a dedicated health care professional to support transition of patients home. This role focuses on coordinating care, communicating with patients, families, and care teams to ensure safe, effective discharge planning.

You will apply clinical guidelines, educate communities about services, and help navigate federal, state, and local regulations. A valid license and home health experience are preferred to deliver compassionate, compliant care.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where they practice.
• At least one year of home care experience.
• Knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions to apprise them of Enhabit Home Health & Hospice services.
• Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency.
• Serve as a public awareness representative and will be responsible for education related to provided services.

Education

Graduate of an approved school of clinical education

Licensed in the state where they practice

At least one year of home care experience

Knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations

Basic technology skills, including operation of a mobile device

• Must have basic demonstrated technology skills, including operation of a mobile device

Education and Experience, Preferred

• Demonstrated experience with and knowledge of hospice is preferred

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
