Job summary

Enhabit Home Health & Hospice is seeking a position focused on coordinating patient-centered transitions and external outreach within the Georgia region. The role emphasizes collaboration with patients, families, physicians, and care facilities to ensure seamless access to Enhabit services.

Qualifications

• Graduate of an approved school of clinical education.
• Licensed in the state where you practice.
• At least one year experience in home care.
• Knowledge of federal, state, and local laws and regulatory guidelines governing hospice operations.
• Basic technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities and professional contacts to inform on Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines, protocols, and metrics to promote quality and efficiency.

Education

Graduate of approved school of clinical education

Licensed in state of practice

At least 1 year home care experience

Knowledge of hospice regulations (federal/state/local)

Basic technology skills; mobile device operation

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible foreducation related to provided services.

Qualifications

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education and experience, preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
