Job summary

Enhabit Home Health & Hospice in Georgia seeks a licensed clinician to represent the branch in external transitional care activities, educate patients and families, and promote the availability of Enhabit’s home health and hospice services.

The role requires state licensure, at least one year in home care, a valid driver’s license, and reliable transportation. Knowledge of applicable laws and comfort with mobile documentation are essential.

Qualifications

• Must have graduated from an approved school of clinical education.
• Must be licensed in the state where they practice.
• Must have at least one year experience in home care.
• Must understand federal, state, and local laws governing hospice operations.
• Must have basic technology skills including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, hospitals, and related health groups.
• Develop patient-centered transition plans by integrating guidelines and protocols.
• Serve as a public awareness representative and educate stakeholders about Enhabit services.

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
