Job summary

Enhabit Home Health & Hospice in New Mexico seeks a field-based coordinator to support patient-centered transitions and collaborate with families, physicians, hospitals, and community partners. You will promote Enhabit services by applying clinical guidelines and efficient care pathways, ensuring smooth transitions.

The role requires a clinical education background, state licensure, and at least one year in home care, plus strong technology skills and a valid driver's license.

Qualifications

• Must possess basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Represent the branch in external transitional care activities with patients, families, physicians, and hospitals to promote Enhabit Home Health & Hospice services.
• Develop patient-centered transition plans by integrating clinical guidelines and protocols to improve quality and efficiency.
• Serve as a public awareness representative and educate about provided services.

Responsibilities

Represent the branch in external transitional care activities, and in professional contacts with patients, families, physicians, hospitals, facilities, senior living communities, professional associations, and similar health groups and institutions, to apprise them of the availability of Enhabit Home Health & Hospice services. Integrate clinical guidelines, protocols, and other metrics to develop patient-centered transition plans that promote quality and efficiency. Serve as a public awareness representative and will be responsible for education related to provided services.

Education and experience, essential

• Must be a graduate of an approved school of clinical education
• Must be licensed in the state where they currently practice.
• Must have at least one year experience in home care.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of a hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• Demonstrated experience with and knowledge of hospice is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
