Job summary

AccentCare, Inc. is seeking a Hospice Team Physician in San Bernardino, CA to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team.

The role collaborates with nurses, social workers, chaplains, and other professionals to deliver compassionate, patient-centered hospice care. The physician will manage symptom control, review medications, and participate in plans of care while ensuring compliance with hospice regulations and Medicare requirements.

Qualifications

• Licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in practicing states.
• Active DEA license and valid driver’s license.
• Strong understanding of primary medical care and hospice care principles.
• Familiarity with hospice regulations and Medicare requirements.
• Board certification in Hospice and Palliative Medicine preferred.
• Five+ years of clinical experience preferred; oversight of clinical programs preferred.

Responsibilities

• Serve as clinical leader within the IDG, contributing to care planning and quality improvement.
• Provide medical care and guidance for hospice patients, including symptom management and med review.
• Develop, review, and update Plans of Care to meet patient and family needs.
• Complete certifications, recertifications, and face-to-face attestations per regulations.
• Collaborate with Attending Physicians and other providers on care plans and decisions.
• Participate in on-call coverage to ensure 24/7 physician availability and support.
• Support hospice admissions, recertifications, revocations, and discharges.
• Maintain accurate, timely, and compliant medical documentation.

AccentCare is seeking a dedicated and collaborative Hospice Team Physician to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team. This physician will play a vital role in supporting patients with terminal illnesses, partnering with attending physicians, and ensuring the delivery of high-quality, patient-centered hospice care.

This position offers the opportunity to work closely with nurses, social workers, chaplains, and other healthcare professionals while making a meaningful impact on patients and families during critical moments in their healthcare journey.

What You Need to Know

Key Responsibilities:

• Serve as a clinical leader within the Interdisciplinary Group (IDG), contributing to care planning, team discussions, and quality improvement efforts.
• Provide medical care and guidance for hospice patients, including symptom management, medication review, and support
• Participate in developing, reviewing, and updating individualized Plans of Care to ensure patient and family needs are met.
• Complete required certifications, recertifications, and face‑to‑face encounter attestations in accordance with hospice regulations.
• Collaborate with Attending Physicians, referring providers, patients, and families regarding care plans, prognosis, and medical decisions.
• Participate in on‑call coverage to ensure continuous (24/7) physician availability and provide timely clinical support.
• Support appropriate hospice admissions, recertifications, revocations, and discharges.
• Maintain accurate, timely, and compliant medical documentation.

If you meet these qualifications, we would love to meet you:

• Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
• Maintains active DEA license
• Valid driver’s license
• Strong understanding of primary medical care principles.
• Working knowledge of hospice and palliative care, including pain and symptom management.
• Familiarity with hospice regulations and Medicare requirements.
• Strong clinical judgment and decision‑making skills.
• Ability to collaborate effectively with Attending Physicians and interdisciplinary teams.
• Excellent communication skills for interaction with patients, families, and providers.
• Ability to manage complex clinical and ethical situations in end‑of‑life care.
• (Preferred) Board certification in Hospice and Palliative Medicine
• (Preferred) Five or more years of clinical experience
• (Preferred) Oversight of clinical program(s)
