Job summary

AccentCare is seeking a dedicated Hospice Team Physician to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team. This physician will support patients with terminal illnesses, partnering with attending physicians to ensure high-quality, patient-centered care.

The position offers a hybrid schedule in San Diego, CA with a salary of $67,500 - $75,000 annually for 8 hours per week.

Qualifications

• Must be a licensed MD or DO in the state of practice.
• Active DEA license required.
• Valid driver's license.
• Strong understanding of hospice and palliative care.
• Familiarity with hospice regulations and Medicare requirements.
• Board certification in Hospice and Palliative Medicine preferred.
• Five+ years of clinical experience and program oversight (preferred).

Responsibilities

• Serve as clinical leader within the Interdisciplinary Group (IDG) for care planning.
• Provide medical care for hospice patients including symptom management.
• Develop and update individual Plans of Care with the team.
• Complete certifications and face-to-face attestations per regulations.
• Collaborate with Attending Physicians and families on care plans.
• Participate in on-call coverage for 24/7 physician availability.
• Support admissions, recertifications, revocations and discharges.
• Maintain accurate medical documentation in a timely manner.

What You Need to Know

AccentCare is seeking a dedicated and collaborative Hospice Team Physician to provide clinical leadership and medical expertise as part of an interdisciplinary hospice care team. This physician will play a vital role in supporting patients with terminal illnesses, partnering with attending physicians, and ensuring the delivery of high-quality, patient-centered hospice care.

This position offers the opportunity to work closely with nurses, social workers, chaplains, and other healthcare professionals while making a meaningful impact on patients and families during critical moments in their healthcare journey.

Key Responsibilities:

• Serve as a clinical leader within the Interdisciplinary Group (IDG), contributing to care planning, team discussions, and quality improvement efforts.
• Provide medical care and guidance for hospice patients, including symptom management, medication review, and support
• Participate in developing, reviewing, and updating individualized Plans of Care to ensure patient and family needs are met.
• Complete required certifications, recertifications, and face-to-face encounter attestations in accordance with hospice regulations.
• Collaborate with Attending Physicians, referring providers, patients, and families regarding care plans, prognosis, and medical decisions.
• Participate in on-call coverage to ensure continuous (24/7) physician availability and provide timely clinical support.
• Support appropriate hospice admissions, recertifications, revocations, and discharges.
• Maintain accurate, timely, and compliant medical documentation.

If you meet these qualifications, we would love to meet you:

• Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
• Maintains active DEA license
• Valid driver's license
• Strong understanding of primary medical care principles.
• Working knowledge of hospice and palliative care, including pain and symptom management.
• Familiarity with hospice regulations and Medicare requirements.
• Strong clinical judgment and decision-making skills.
• Ability to collaborate effectively with Attending Physicians and interdisciplinary teams.
• Excellent communication skills for interaction with patients, families, and providers.
• Ability to manage complex clinical and ethical situations in end-of-life care.
• (Preferred) Board certification in Hospice and Palliative Medicine
• (Preferred) Five or more years of clinical experience
• (Preferred) Oversight of clinical program(s)
