Qualifications

Two years supervisory experience in a healthcare setting.
Knowledge of primary medical and nursing care with palliative care understanding.
Familiar with interdisciplinary teams and patient-centered planning.
Proficient use of healthcare information systems (e.g., Vx) for documentation and reports.

Responsibilities

Directs all patient care services for the team as an integrated part of the interdisciplinary plan of care.
Reviews admissions and alive discharges to ensure eligibility and appropriateness.
Collaborates with team and attending physicians on pain and symptom management.
Ensures complete data and documentation for timely recertification decisions.
Monitors service levels (staffing, medications, supplies) and maintains 24/7 team coordination.
Facilitates team meetings and interdisciplinary input for care plan updates.
Participates in on-call rotation.
