Job summary

Agape Care Group is seeking a dedicated Team Coordinator (PRN Week Days) in Lexington, SC. The role focuses on answering calls, coordinating daily workflows, assigning clinician visits, and supporting interdisciplinary meetings to ensure seamless patient care.

The candidate will manage office inventory, support clinical leaders, and contribute to a positive, client-centered environment with opportunities for professional growth.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: Associate degree preferred.
• Experience: Minimum of 1-year experience in a health care setting. Scheduling and office manager experience preferred
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs

Responsibilities

• Answer all incoming calls and route to appropriate teams.
• Process daily workflow and schedule clinician visits.
• Prepare interdisciplinary meetings and support clinical leaders.
• Manage and maintain office inventory and medical supplies.
• Ensure smooth operation of the agency so patients receive needed care.
