Mercy Medical Center Home Health and Hospice is seeking a Hospice Aide I to provide compassionate personal care to patients at the end of life under RN supervision. You will assist with ADLs, provide comfort, and support families in Canton, OH.

The role requires current CNA/STNA certification, at least 1 year of related experience, and ability to read/write English. Valid driver's license and reliable transportation are required to serve patient homes.

Qualifications

  • Current CNA/STNA certification in the state of practice.
  • Minimum of 1 year of experience providing personal care in hospice, home health, or long-term care preferred.
  • Ability to read, write, and communicate effectively in English.

Responsabilités

  • Provide personal care to patients under the supervision of the RN according to the plan of care.
  • Assist with ADLs, including bathing, dressing, grooming, toileting, and mobility support.
  • Offer emotional and physical comfort to patients and families during end-of-life care.
  • Observe and document patient condition, changes, and care provided; report changes to the RN.
  • Maintain a clean and safe environment, follow HIPAA, company, and regulatory guidelines.
  • Perform other duties as assigned.

Working closely under the supervision of a registered nurse, the hospice aide helps with personal care tasks like bathing, dressing, and grooming to ensure patients feel comfortable, safe, and supported. In addition to providing physical care, the hospice aide offers emotional comfort and companionship to both patients and their families. This role is essential in helping people live their final days with dignity, peace, and as much comfort as possible.KEY RESPONSIBILITIESProvide personal care to patients in accordance with the plan of care and under the supervision of the RNAssist with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility supportOffer emotional and physical comfort to patients and caregivers during end-of-life careObserve and document patient condition, changes, and care provided; report changes to the RNMaintain a clean and safe environment for the patient, including light housekeeping related to patient careUse infection control techniques and universal precautions consistentlyProvide support to the patient and family within their cultural and personal belief systemMaintain confidentiality and comply with all HIPAA, company, and regulatory guidelinesParticipate in interdisciplinary team meetings and in-service education as requiredMaintain accurate documentation in accordance with agency standardsPerform other duties as assignedQUALIFICATIONSCurrent certification as a Nurse Aide (CNA, STNA, or equivalent) in the state of practiceMinimum of 1 year of experience providing personal care in hospice, home health, or long-term care preferredAbility to read, write, and communicate effectively in EnglishCompassionate, dependable, and respectful in all patient interactionsValid driver’s license, reliable transportation, and current auto insurance requiredAbility to work independently in patient homes and travel within assigned territoryEDUCATION AND CREDENTIALSHigh school diploma or equivalent required#INDMER3We’ll help you put your passion for patient care to work.
