You will collaborate with nurses, social workers, chaplains, volunteers, and a dedicated STNA leadership team to implement care plans and maintain safety standards.

Qualifications

Responsibilities

• Provide resident care per assessments and care plans under supervision of Case Manager/DON.
• Coordinate care with licensed staff to ensure appropriate care.
• Follow nursing policies, procedures, and safety guidelines.
• Support a 1-1 relationship with residents while collaborating with care team.
