The role supports care plans and coordinates with nurses, social workers, chaplains, volunteers, and the patient care team to ensure high-quality outcomes.

Qualifications

• State Tested Nursing Assistant certification in the state(s) of practice.
• CPR certification.
• Commitment to improving quality of life and community collaboration.
• Ability to collaborate daily with nurses, social workers, chaplains and volunteers.
• Willing to drive with reliable transportation and a valid driver’s license.

Responsibilities

• Provide resident care in accordance with assessments and care plans.
