Qualifications

• Minimum 16 years of age.
• Active State Tested Nursing Assistant (STNA) / Certified Nursing Assistant (CNA) certification.
• Listed on the state nurse aide registry in good standing.
• Current BLS.
• Passionate care to terminally ill patients.
• Strong communication and observational skills.

Responsibilities

• Provide direct personal care to hospice patients in home settings.
• Assist with activities of daily living (ADLs) including bathing, grooming, oral care, dressing, and toileting.
• Support patient comfort and safety, including proper use of medical equipment (oxygen, bed rails, etc.).
• Perform skin care and positioning to prevent breakdown and promote comfort.
• Assist with meal preparation, feeding, and light household tasks as assigned.
• Take and document vital signs (temperature, pulse, respirations, blood pressure) as directed.
• Observe and report changes in patient condition to the RN Case Manager.
• Provide emotional support and companionship to patients and families.
• Offer respite care for caregivers when needed.
• Maintain accurate, timely documentation in compliance with hospice standards.
