Job summary
Care Initiatives Hospice seeks a PRN Hospice Spiritual Care Coordinator to deliver compassionate spiritual care across Council Bluffs, Avoca, Dunlap, and nearby communities. You will meet patients where they are, providing counseling and bereavement support as part of an interdisciplinary team.
Responsibilities include comprehensive spiritual assessments, tailoring interventions to patient beliefs, and ensuring alignment with the care plan.
Qualifications
- Master of Divinity degree with ordination; licensure acceptable if ordination not practiced.
- Baccalaureate degree in social work with MSW supervision, or in psychology/sociology/counseling.
- Demonstrated ability to provide spiritual care and bereavement support in hospice settings.
Responsibilities
- Arrange or provide counseling to meet spiritual needs of patients, families, and caregivers.
- Conduct comprehensive spiritual assessments and bereavement risk assessments.
- Identify interventions and goals for the patient care plan.
- Collaborate with the interdisciplinary team to ensure services align with care plan.
