Job summary

Care Initiatives Hospice is seeking a PRN Visit Nurse to provide patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities. The RN will meet patients where they are, delivering compassionate, high-quality care in home, long-term care, assisted living, or hospital settings.

We support a holistic approach to addressing physical, emotional, and spiritual needs, working closely with families. Applicants should have an RN license with at least two years of nursing experience and CPR

Qualifications

• RN with two years of recent clinical nursing experience
• Current, valid CPR certification
• Demonstrated patience and a supportive approach to patient, family, and caregiver support
• Willingness to provide care in home and facility settings

Responsibilities

• Collaborate with the physician to oversee and implement personalized care plans with input from interdisciplinary teams
• Provide high-quality patient care ensuring an exceptional patient experience
• Perform initial and comprehensive assessments in accordance with policies and procedures
• Maintain clear, concise, and accurate clinical documentation
• Assess need for plan of care updates with patient and family involvement
